This is an introduction of the Illinois Department on Aging's Community Care Program- a medicaid waiver program. This program evaluates the needs of seniors to identify programs and resources to allow seniors to remain living at home as long as safely possible. 

Laura Gnutek

For over 20 years, Laura Gnutek, has worked with seniors and individuals with disabilities in the areas of direct care, case management, and advocacy. She has been with Senior Services Associates for 10 years as a Care Coordination Unit Supervisor and Director of Case Management. Laura currently oversees the Community Care Program as the Director of Case Management in Kane, Kendall, and McHenry Counties, as well as oversees the Choices for Care, Information and Assistance, and Caregiver Counseling and Support programs. Senior Services Associates, Inc. is a non-profit agency dedicated to sustaining and improving the quality of life for individuals age 60 and older, the disabled, and their caregivers by providing access to the social services they need.
